Job Description
Summary: Provides personal care and assistance for individuals served according to the individualized plans of care, under the supervision of a licensed nurse. Schedules and coordinates medical appointments for and transports individuals served to clinic by performing the following duties.
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
- Satisfactory attendance as assigned by Director of Nursing, Nurse Manager or Nursing Supervisor
- Complies with Nursing Policy and Procedures.
- Adheres to facility infection control standards.
- Protects and promotes rights of individuals served.
- Assists nursing staff as directed.
- Obtains and records vital signs on individuals served as needed.
- Obtains and records monthly weights on individuals served.
- Assists individuals served with activities of daily living including feeding and documentation, transporting in wheelchairs, vital sign monitoring, etc.
- Administers gastrostomy tube feedings after passing competency check-off.
- Reports all changes in condition of individuals served to a licensed nurse as soon as possible.
- Schedule routine and annual appointments for individuals served by the program.
- File medical reports, lab work, and other documents in the medical chart.
- Notifies parent/guardian by mail or phone of all appointments and appointment cancellations.
- Transports individuals served to medical appointments. Drives and assists with individuals served on special outings, as available.
- Uses appropriate safety measures when transporting individuals.
- Picks-up and delivers equipment/supplies for nursing and other departments.
- Maintains, inspects, and operates vehicles in accordance with Easterseals Arkansas policies and procedures.
- Completes other duties as assigned in a timely manner.

While performing the duties of this job, employee will comply with all federal, state, and local laws pertaining to Easterseals Arkansas.
Employee has access to all client protected health information within his/her program. Employee will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer.
Supervisory Responsibilities:
No direct reports. No supervisory responsibilities.
Qualifications

Education and/or Experience:
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
Language Skills:
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one situations to clients, and other employees of the organization.
Mathematical Skills:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Reasoning Ability:
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Certificates, Licenses, Registrations:
Valid Arkansas Driver's License
CNA license

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

While performing the duties of this Job, the employee is frequently exposed to moving mechanical parts and be performing tasks involving blood/body fluids. The noise level in the work environment is usually moderate.
